Summary Description: The Skilled Nursing Facility Licensed Practical Nurse (LPN) coordinates and delivers direct and indirect resident care under the supervision of the Registered Nurse. The LPN provides care that meets the psychosocial, physical, and general aspects of care while meeting the communication needs of the resident and family. The LPN shall utilize knowledge of residents’ rights, legal/ethical concepts, communication skills, safety and infection prevention while performing the following: basic nursing skills, basic restorative skills, mental health and psychosocial skills, and nursing team member skills. Participates in resident and family teaching while maintaining a role as the resident’s advocate. The LPN supervises the Certified Nursing Assistant (CNA) staff.
Supervision Received: Reports to the Director of Nursing (SNF).
Responsibilities: 1. Must possess the ability to utilize the nursing process in the provision of nursing care including but not limited to; administering treatments and medications in compliance with the physician’s orders and applicable hospital policy, interpreting diagnostic tests, assessing, and reassessing residents and formulating plan of care within the nursing scope of practice. 2. Provides direct resident care under the supervision of an RN; evaluates outcomes, consults with other health team members as required and adjusts nursing care process as indicated to ensure optimal resident care. 3. Functions as a leader for the CNA’s; delegates in a manner that contributes to the excellence of the nursing unit. 4. Assesses residents on admission and on a weekly basis as well as when conditions change. Documents appropriately including Minimum Data Sets under the supervision of a Registered Nurse. 5. Performs resident care responsibilities considering needs specific to the standard of care for residents’ age, developmental status, mental and or physical abilities. 6. Ability to adequately assess and reassess pain. Utilize appropriate pain management techniques. Appropriately educates the resident and family regarding pain management. 7. Administers medication as ordered by health care providers while abiding by state licensure practices and maintaining proper documentation in the resident record. 8. Demonstrates knowledge of medications and their correct administration based on age or weight of the resident and their clinical condition. 9. Provides direct patient care, according to physician’s orders and individual resident’s needs, including administration of medications, treatments, therapies and performs and assists with ADL’s, in accordance with recognized standards of practice and organizational standards. 10. Identifies and reports changes in condition to the attending or on call physician, the DON, and the responsible family member. To include documentation of this change in the progress notes for the resident’s medical record. 11. Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors. 12. Communicates appropriately and clearly to Director of Nursing, co-workers, and providers. 13. Possesses the ability to assess data reflective of the resident’s condition and interpret the appropriate information needed to identify each resident’s needs. 14. Maintains and reviews resident’s electronic and paper records, and charts and all pertinent information while using proper charting practices. Must document/chart observations and other related data to the clinical condition of the resident in the nursing notes in the EMR. 15. Accurately communicates report of resident status to the appropriate healthcare workers in accordance with the medical center policy. 16. Assist health care providers with examinations and treatment procedures. Obtains laboratory samples in accordance with policies and procedures to ensure correct sample, correct identification of sample and correct testing ordered. Assists radiology with the transport of residents for imaging if the level of care requires nursing care or interventions. 17. Demonstrates ability to perform treatments and provide services to level of licensure. 18. Demonstrates the ability to assist the providers with procedures and perform services requiring technical and manual skills. Ensures that medical and surgical asepsis is carried out during treatments and special procedures. 19. Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of residents and their families. Functions as liaison between administration, residents, physicians, and other healthcare providers. 20. Maintains established departmental policies and procedures, objectives, performance improvement program, safety, environmental and infection prevention, and control standards. 21. Demonstrates the ability to be flexible, organized and function under stressful situations. 22. Assists in cost containment through the appropriate use and conservation of supplies and equipment. 23. Maintains proficiency in laboratory procedures as required in the Laboratory Point of Care policy manual. 24. Communicates the mission, ethics, and goals of the hospital. Complies with all organizational policies regarding ethical business practices. Represents the organization in a positive and professional manner. 25. Demonstrates responsibility for knowing and adhering to all District and departmental policies and procedures. 26. Understands the importance of resident confidentiality and HIPAA rules and regulations. 27. Other Duties as assigned.
Qualifications: 1. Diploma graduate of a Practical Nurse accredited program of study. 2. Current State of Wyoming Practical Nurse License without restrictions. 3. Minimum of one-year practical nursing experience. Applicants with emergency department and outpatient clinic experience preferred. 4. Current certification in Basic Life Support. 5. Preferred basic IV Certification through the State of Wyoming.
Knowledge, Skills, and Abilities: 1. Possesses a genuine interest and concern for adults in later years of life. 2. Demonstrates excellent verbal and written communication skills. 3. Knowledge of long-term care procedures and goals. 4. Ability to work as a member of the "health care team." 5. Knowledge of good body mechanics and proper lifting techniques. 6. Knowledge of performing and following infection prevention and control procedures. 7. Ability to follow instructions and ask questions as appropriate. 8. Demonstrates the ability to remain calm in emergency and/or stressful situations. 9. Ability to communicate clearly and effectively with residents, visitors, and coworkers.
Working Conditions: 1. Subject to varying situations including ability to understand and respond appropriately, effectively, and sensitively to special population groups, including those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, etc. 2. Frequent contact with a variety of people including prisoners and people with mental, emotional, and physical disabilities. 3. Medical office, exam room, laboratory, radiology, emergency room and professional practice settings. Frequent exposure to communicable diseases and other conditions common to rural clinic environments. 4. The work environment involves exposure to unusual elements, potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. 5. May require travel between District sites.
Physical Demands: Varied activities include standing, walking, reaching, bending, and lifting. Requires full range of body motion including handling and lifting patients. Also requires manual and finger dexterity, and eye-hand coordination. It requires standing and walking for extensive periods of time. Occasionally lifts and carry items weighing up to fifty pounds. Requires corrected vision and hearing to normal range. This position frequently requires working under stressful conditions and irregular hours. Year-round highway travel between clinics and other facilities throughout the district is required.
